LAB 1058: Workplace Committee Report

  • Completed by JHSC
  • Annual submission by March 1st
    • Contains JHSC information
    • Covers monthly health and safety statistics such as meetings, work refusals, inquiries, investigations, inspection and programs being monitored.
    • Use information gathered from various documentation such as meeting minutes, inspections reports, and investigation reports (LAB 1070) and work refusals(Lab 1184).

LAB 1009: Employer’s Annual Hazardous Occurrence Report

    • Annual Submission by March 1st
    • Contains the information regarding the injury statistics and employment data of each location
    • Use information from LAB 1058 to complete this form
      • Note: this information form should match with the information on LAB 1058

LAB 1206: Employer’s Annual Harassment and Violence Occurrence Report

  • Annually submitted by March 1st
  • Contains company’s details, number of occurrences received, resolved related to workplace Harassment and Violence and/or based on discrimination grounds
  • Identifies the principal and responding party involved in the occurrence
